• Charger retention feature is worn or obstructed
• Pack was not pushed to the positive retention point
• Dry debris prevents full latch engagement
• Cord strain or wall angle is disturbing the seated pack

1. A sound pack remains fully seated through a gentle no-force retention check and a normal charge, or the faulty complete item is removed from service.
